Myoga-kai Social Welfare Corporation (hereinafter referred to as "the Corporation") considers the proper handling of personal information of users and others to be a serious responsibility for those involved in providing care services.

We hereby declare that we will strive to handle the personal information of our users and others that we hold in a proper and appropriate manner, and that in order to gain the trust of society at large, we will establish our own rules and systems, comply with laws and other relevant regulations related to personal information and the guidelines of the Ministry of Health, Labour and Welfare, and protect personal information.

1. Appropriate acquisition, management, use, disclosure, and outsourcing of personal information ① When acquiring personal information, we will clearly state the purpose of use, acquire only the necessary information, notify or publicly announce the purpose of use, and use the information only within that scope.
② We will obtain the individual's consent before acquiring, using, or providing their personal information to a third party.
③ When our organization outsources medical and nursing care-related services, we select businesses that understand the spirit of the Personal Information Protection Act and guidelines and act accordingly, and we provide information only after concluding a confidentiality agreement, and we appropriately supervise the outsourced parties.

2. Measures to ensure the security of personal information ① Our organization will establish regulations concerning personal information protection and provide necessary training on an ongoing basis to ensure that all officers and employees are fully aware of our efforts to protect personal information.

② To prevent and correct unauthorized access to personal information, as well as leakage, loss, or damage of personal information, our organization will establish regulations and strive to implement safety measures.

3. Response to requests for disclosure, correction, updating, suspension of use, deletion, etc. of personal information

Our organization will promptly respond to requests from individuals regarding the disclosure, correction, updating, suspension of use, or deletion of their personal information. If you wish to make such a request, please contact the Hamana-ko Garden office at 053-485-4116.

4. Handling of complaints

Our organization strives to handle complaints regarding the handling of personal information appropriately and promptly. Our personal information protection policy will be published in writing on our bulletin board.
